Overview

Responsible for the day-to-day oversight of the Hospital Liaisons, Admission RNs, LVNs, and MSWs. Responsible for adequate staffing to ensure timely patient access to hospice services seven days a week.

Initiates establish, develops, and maintains professional relationships between hospice and various referral sources, e.g., hospitals, hospitalists, facilities, and independent medical clinics/offices, to ensure a consistent base and flow of patient referrals. Collaborate with the Medical Director on any high acuity referrals or admissions. Provides direct supervision of Hospital Liaisons, Placement Coordinators, Admission RNs, LVNs and MSWs as evidenced by ongoing ride-along and coaching. Works collaboratively daily with the Access Center Manager or Supervisor to ensure timely same-day hospice admissions. Responsible for working closely with the Director of Sales and Marketing, and Regional Liaisons to ensure growth and customer service objectives are met. Ensures professional handoff of all service recovery concerns the appropriate Regional Liaisons. Displays an ongoing commitment to excellence in customer

service to both internal and external customers.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. 

Salary: $120,000-$140,00 annually. Subject to experience and qualifications.

Work Location: Escondido, CA

Travel: Travel will be required for events

Responsibilities

1. Ensures compliance with State and Federal regulations regarding obtaining and processing of referral

information and patient admissions to hospice.

2. Works with the marketing team in nurturing relationships and building a referral base.

3. Serves as Clinical Resource for the Access Center reviewing referrals and pending inquiries.

4. Ensures timely effective follow-up of all pending referrals.

5. Implements protocols to assure compliance with all federal, state and local health care regulations as

they affect the provision of clinical services.

6. Responsible for providing day-to-day oversight of the Hospital Liaisons, Admission RNs, LVNs, and

MSWs.

7. Oversees the orientation and mentoring of new Hospital Liaisons, Admission RNs, LVNs, and MSWs.

8. Responsible for updating and maintaining the liaison resource/orientation material.

9. Oversees all hiring, counseling, and terminations. Collaborates with Human Resources to ensure

competitive recruitment and retention activities.

10. Responsible for staff compliance with all required clinical and administrative deadlines.

11. Responsible for timely all staff evaluations and performance plans.

12. Ensures the coordination of services for discharge of patients from hospitals onto T.E.H.’s program

of care.

13. Collaboratively works with the Access Center Manager or Supervisor, Director of Sales and Marketing

along with the Regional Liaison Team to troubleshoot real or perceived problems identified by referral

sources and resolve customer complaints.

14. Demonstrates knowledge and shares expertise in aggressive symptom management as well as

assessment of patient/family psychosocial and spiritual needs, implementing appropriate level. and plan

of care with team, collaborating with the program Medical Director

Required Qualifications:

  • Current Registered Nurse (RN) license in good standing
  • Minimum of 2 years in the last 5 experience in hospice, palliative care, home health, or community.
  • Strong knowledge of Medicare hospice benefit and Conditions of Participation (CoPs).
  • Proficiency with EMR systems (e.g., WellSky, MatrixCare, Netsmart) and ability to use reporting and workflow tools.
  • Demonstrated ability to manage a team, drive performance, and lead through change.
  • Excellent communication, organizational, and critical thinking skills.

Preferred Qualifications:

  • Bachelor's degree in nursing, healthcare administration, or related field.
  • Experience with remote/telehealth admissions and after-hours workflows.
  • Bilingual or multilingual skills.
  • Certification in hospice and palliative care (CHPN, CHPCA, or equivalent).
  • Minimum of 3 years supervisory or management experience, preferably in hospice or a related healthcare setting (acute care)
